Abstract

Objective:To examine the relationship between trimethoprim-sulfamethoxazole (TMPSMX) clearance and renal function in adult patients with chronic kidney disease (CKD) and to determine if TMPSMX clearance is a determinant of renal function.

Methods:Patients with a history of CKD, as defined by the Renal Disease Evaluation and Therapy Program, were enrolled in a prospective registry of CKD patients on dialysis. Patient and renal function were monitored by collecting a urine sample at 2, 4, 8, 12, and 24 months.

Results:There was no significant difference in the rate of urinary clearance between patients with and without renal dysfunction (creatinine clearance [CrCl] ≥ 1.5 ml/min and CrCl ≥ 2.2), as determined by creatinine clearance [CrCl] ≥ 2.2 ml/min and CrCl ≥ 2.2, respectively. TMPSMX clearance decreased significantly with increasing renal function. The median TMPSMX clearance was 3.5 ml/min and 3.0 ml/min in the kidney patients with and without renal dysfunction, respectively.

Conclusions:TMPSMX clearance was significantly lower in renal dialysis patients with a higher renal function, and renal function was associated with a decrease in TMPSMX clearance.
